New Team/Parent Registration Party May 28th 6:00-9:00pm-refreshments will be served.  Uniforms will be available to try on for proper sizing.

What do I need to bring:

  1) Copy of Birth Certificate  (NEW PLAYERS ONLY)
  2) One inch by one inch picture for player card
  3) Medical Waiver needs to be completed and notarized.  We will have a notary available
